Alternate Relationship Plans

An Alternate Relationship Plan (ARP) is an alternative to the traditional fee-for-service method of physician payment. ARPs provide physicians with set remuneration amounts in exchange for delivering specific levels of service or time.

The Long Term Care Alternate Relationship Plan program focuses on medical service delivery in long term care Centres. The ARP model created for this program will generate innovative methods for the delivery of care through collaborative partnerships, coordination of care, studious evaluation, and stakeholder dedication. Intercare Corporate Group Inc. is the owner/operator of the long term care centres where the program is implemented.

  • ARP Program
    Initiation July 1 2008
  • Associate Team Leader (ATL)
    A registered nurse is in a leadership role at each site. He or she plans and coordinates physician visits to strengthen resident/patient interaction and clinical management.
  • Evaluation Framework
    Developed for continuity of program review and for coordination of data collection and measurement of project performance.

  • Quality Improvement Initiative
    Falls and Fractures – A physician-led initiative, with care centre staff, will complete a focused quality improvement study on hip fractures at Intercare care centres. Best practices and research will be reviewed as well as site specific data collected to generate recommendations that will lead to a decreased number of falls.
    Hospital Transfers – To better understand hospital transfers from long term care, a physician lead review team will examine patient cases from Intercare care centres. This study will identify opportunities to improve hospital transfer process and case management leading to a decrease in the number of hospital transfers.
